Symptoms of disease - violation of uterine blood flow
Violation of uterine blood flow (utero-placental and fetal-placental blood flow) in a single functional system mother-placenta-fetus is playing a major role in the pathogenesis of placental insufficiency, which is one of the leading causes of perinatal morbidity and mortality. Many works show the fundamental role of the syndrome of the hemodynamic placental insufficiency in the clinic of miscarriage.
Bloodstream placenta includes two closely related, but not communicating between the systems of blood circulation - fruit and placental. Three-dimensional ultrasound examination gives the opportunity to form an idea of the three-dimensional organization of the system of blood vessels in the placenta. When assessing the results of 3D-study of a selected zone of the placenta is necessary to pay attention to the character of the distribution of the vascular component and the organization of vessels in the area, that allows to obtain the data necessary for understanding the specifics of its physiology. Three-dimensional dopplerometry study gives spatial relationships fetal-placental and uterine-placental system of blood circulation, as well as diagnose violation of placental blood flow.
Placental insufficiency is manifested in violation of transport, food, endocrine, metabolic and other essential functions of the placenta, which leads to a decrease in its ability to maintain adequate sharing between organisms of the mother and the fetus. The basic sign of fetoplacental dysfunction (41,9% of all cases of chronic placental insufficiency) are the most prevalent disorders of the blood flow in the arteries of the umbilical cord, chorionic records and reference villus confirmed by the results of dopplerometry.
The outcome of decompensirovanna fetoplacental insufficiency is intrauterine fetal development, due to the oppression of the respiratory, hormonal and trophic functions of the placenta. The degree of severity of intrauterine fetal growth, as a rule, correlate with the severity of placental insufficiency.
Which diseases there is violation of uterine blood flow
The method of ultrasonic dopplerometry, with the help of which carry out the direct measurement of blood flow in various vascular zones of the system mother-placenta-fetus in dynamics allows to assess the status of placental blood flow and, therefore, has an important diagnostic and prognostic value in a group of pregnant with high perinatal risk. Conducted numerous studies prove that a comprehensive assessment of the circulation in the system mother-placenta-fetus allows you to improve the diagnosis and choice of the optimal obstetric tactics at the placental violation.
A classification was developed of the violations of uterine blood flow (utero-placental and fetal-placental blood flow), based on an assessment of the curves of the velocity of blood flow in the uterine arteries and the arteries of the umbilical cord. According to this classification, there are three degrees of severity of hemodynamic disorders:
I degree:
A - violation of the utero-placental blood flow with a normal fetal-placental blood flow.
B - violation of the Feto-placental blood flow with a normal uterine-placental blood flow.
II degree: simultaneous violation of the utero-placental and fetal-placental blood flow, not reaching critical changes (saved of course-diastolic blood flow).
III degree: critical violations of the Feto-placental blood flow (a lack of blood flow or reverse diastolic blood flow) with a normal or reduced maternal-placental blood flow.
Awarded directly proportional with a high coefficient of correlation dependence between the degree of hemodynamic disorders in the system mother-placenta-fetus and frequency of delay of development of the fetus, fetal hypoxia, prompt delivery by cesarean section, the dire state of the newborn and perinatal losses. It should be noted that under dynamic observation is not noted normalization or an improvement in hemodynamics in IA, II and III degree of the violation of utero-placental-fetal blood flow. The normalization of the Feto-placental blood flow was noted only when I B degree, usually in pregnant women with the threat of termination of pregnancy.
